After launching, sunk before completion May 15, 1969 when water entered forward compartment. Raised three days later and completely refitted, earning nickname "Mud Puppy". Commissioned September 9, 1972. Decommissioned May 29, 1992. Scrapping/recycling completed October 18, 1994.
